Policies

Appointments

We see all patients by appointment. You can make an appointment by calling 503.474.2566. If you cannot keep an appointment, please let us know at least 24 hours prior to the appointment so that we may accommodate another patient. Same day appointments are available on a very limited basis. Please call for availability.

Telephone Calls

Telephone calls will be returned by the end of the business day. After hours, the on-call medical provider is available to help you with urgent medical needs. Call 1.888.344.5874 and have the on-call medical provider paged. If you have a life threatening illness or injury, call 911.

Referral Requests

It is our policy to treat all illnesses and injuries as appropriate. Referrals are given at the discretion of the primary care provider. If you are referred to a specialist, be sure and verify with your insurance company that the provider is covered under your plan. If you plan requires you to have a referral prior to being seen by one of our providers, it is your responsibility to obtain the referral or we may not be able to treat you.

Patient Rights

Patients have a Bill of Rights that is posted in our lobby. Patients who refuse to follow medical advise or exhibit unacceptable behavior may be dismissed from our practice. All patients are required to provide picture identification.

Other

We do not admit or manage patients in the Hospital. Instead, Hospitalists (internists and pediatricians) will manage your care should you require hospitalization. We do manage patients in nursing homes or assisted living facilities if they have enough mobility to come to the office for appointments.

Payment Terms

We make every effort to keep the cost of your medical care down. You can help by following our office financial policies. Insurance co-payments and deductibles are to be paid at the time of service. We offer a discount for private pay individuals if paid in full at time of service. If you are unable to pay at the time of service, depending upon your payment history, we may allow a short term monthly payment plan, however the discount policy does not apply.
